Fill out a Motor Vehicle Release webform below, or send a completed PDF Form to transportation@csuohio.edu. These should be submitted a minimum of 2-weeks prior to a requested reservation. Please note: in some cases, including MVR requests with an out-of-state license, it may be necessary for CSU's Department of Human Resources to run an in-depth screening. There is a $23 cost to run the background check this way, which is typically charged to the department or student organization's account. Once submitted, these screenings may take 3-5 days for approval.
